General business hours (dependent on my theatre schedule) are:

Urgent / Trauma cases will be seen on the day wherever possible or at the next available consulting session.

As a specialist clinic, patients need a referral from their General Practitioner (Family Doctor) or other doctor to be eligible for a rebate from Medicare. Referrals are welcome from Allied Health professionals (i.e. Physiotherapist) but they do not attract a Medicare Rebate.

When you come for your appointment please bring:

• For new patients – Please get this form filled before you come for your first consultation
• Medicare care
• Private health insurance information
• Referral letter
• X-rays, Reports , MRI , CT Scans and any other relevant information
• EOS Imaging
• WorkCover or TAC claim details

Fees

A portion of the consultation fee will be rebatable from Medicare. Consultation fees are to be paid at the time of consultation unless other arrangements are made, by cash, EFTPOS, Visa or MasterCard and cheque (Amex and Diners are not accepted). Please be aware that in most instances, a co-payment is required for surgical procedures.

Private Patients

Being privately insured allows you to choose your preferred surgeon and will allow you the best possible personalised care. However if you choose to pay for a procedure yourself, then we can provide you with an itemised quote for the surgery, anaesthetic, assistance and hospital costs.

We recognise that your time is valuable, and as such we make every effort to run on time. Occasionally emergencies or patients with complex needs require a little extra time and these cause scheduling delays beyond our control. We apologise if we keep you waiting.

WorkCover and TAC Patients

Mr. Justin Lade accepts WorkCover and TAC patients. Such patients are requested to kindly make full payment at the time of consultation.

Evening Appointments

Evening appointments will be arranged for patients who cannot attend the practice during the normal working hours. This facility is strictly restricted to pre-booked patients and we request you to kindly book your appointments before you come in for an evening appointment.
